Conception that occurs prior to membership is not eligible for sharing. Newborns must be enrolled within 30 days of birth. Pre-approval required for major procedures or 25% penalty applies. Care Coordination required for pre-planned procedures (cancer, orthopedic surgeries, cardiac procedures, neurosurgeries, dialysis, transplants, specialized wound care, GI procedures, prosthetics). SmartPay is not insurance.
